Communication Passport

The ‘Communication passport’ is designed to be a quick and easy way to give health and social care professionals more information about you to help them provide right care and treatment. It should help them understand what reasonable adjustments they should make for you and how to communicate effectively so your voice is always heard.

You can complete the Communication passport on your own or with support. You might want to talk to different people about what information to include about yourself in each section.

You will need to check the information in the passport is updated from time to time. If your health needs haven’t changed very much you could check the passport once a year.
