The number of oncology patients awaiting a Harm Review from the trust.

Worcestershire Acute Hospitals NHS Trust do harm review per speciality like Breast, colorectal etc. and not Oncology on its own. 

Does the trust have a dedicated IT system to support the harm review process?

No this is a manual spreadsheet process, all breeches are reported on the Somerset Cancer Register and All harm reviews will be recorded on our risk system Datix.
