No, it is not essential. If you cannot complete your Synopsis questionnaire then you will be contacted by a nurse closer to the time. They will help you go through the questionnaire in person or over the telephone. This will happen automatically for you so please don’t worry that you won’t get your procedure if you are unable to complete the questionnaire.
Synopsis is designed to be user friendly and so we would ask you to try to complete it when possible. This may help to reduce the time you spend in the pre-assessment clinic with us. You may wish to ask a trusted family member or friend to assist you if you feel able to share medical details. If you do not have anyone to help please don’t worry, it does not mean your operation will be cancelled.

If you are new to Synopsis please enter the website: www.synopsishome.com.
Then click on ‘SIGN UP’. You will need to enter your name, date of birth, email address and mobile telephone number. You will also need to set a password.
A confirmation link will be sent to your email account to complete the registration process.
Do I need to know my ID number or NHS number?

You will need your ‘casenote number’ which is also called your ‘Hospital Number’. This is given as a 10 digit number which begins with 3 of the same digit, i.e. 5550123456. You can find your hospital number on any letter sent to you by the Trust.
The system will not accept your NHS number.

Your questionnaire will be reviewed by a nurse. This will help to decide what type of pre-operative assessment you will need.
Some people will need minimal pre-operative assessment involvement. This is usually people who are fit and healthy having minor operations.
Some people will need a longer pre-operative assessment. This could be a telephone call or a face to face visit.
If you have multiple medical problems you might need to see an anaesthetist and have some extra tests before surgery.

If you are struggling with Synopsis an appointment will be made for a nurse to complete the questionnaire with you during your formal Pre-Operative Assessment.
If you have a clinical query relating to your operation (i.e. you no longer need the operation) you should contact your surgeon's secretary.

If you have already completed a Synopsis questionnaire for us (for example for another operation) then you will need to update it, especially if your medical details have changed.
To do this, please login using the email address and password you registered with originally. You can update your details. Any changes made will be viewable by the assessing team immediately.
