    Why am I being asked to give my medical details

    Before surgery we need to make sure you are in the best possible shape. Part of this is a pre-operative assessment. We need to know your medical details and medications. Synopsis enables you to enter these details in your own time and home. It can save a lot of time and helps us concentrate on the answers and ensure you are properly assessed.

    Unfortunately some surgery waiting lists are long at the moment. Filling out Synopsis does not guarantee a date for surgery. You might still have a long wait before your surgery. However completing your Synopsis questionnaire is essential preparation and can help us detect any problems early on.
  • Do I have to complete Synopsis to have an operation?

    Do I have to complete Synopsis to have an operation?

    No, it is not essential. If you cannot complete your Synopsis questionnaire then you will be contacted by a nurse closer to the time. They will help you go through the questionnaire in person or over the telephone.

    Synopsis is designed to be user friendly and so we would ask you to try to complete it when possible. You may wish to ask a trusted family member or friend to assist you if you feel able to share medical details. If you do not have anyone to help please don’t worry, it does not mean your operation will be cancelled.

  • How do I register with Synopsis?

    How do I register with Synopsis?

    If you are new to Synopsis please enter the website: www.synopsishome.com.

    Then click on ‘SIGN UP’. You will need to enter your name, date of birth, and email address and mobile telephone number. You will also need to set a password.

    A confirmation link will be sent to your email account to complete the registration process.

  • Do I need to know my ID number or NHS number?

    Do I need to know my ID number or NHS number?

    You will need your ‘casenote number’ which is also called your ‘Hospital Number’. This is given as a 10 digit number which begins with 3 of the same digit, i.e. 5550123456.

    The system will not accept your NHS number.

  • How do I share the questionnaire?

    After completing all the questions you will be asked to share the questionnaire with a hospital. Please enter Worcestershire Acute Hospitals and this will ensure the questionnaire is sent back to us.
  • What will happen with the answers?

    What will happen with the answers?

    Your questionnaire will be reviewed by a nurse. This will help to decide how much pre-operative assessment you will need.

    Some people will need minimal pre-operative assessment. This is usually people who are fit and healthy having minor operations.

    Some people will need longer pre-operative assessment. This could be a telephone call or a face to face visit.

    If you have multiple medical problems you might need to see an anaesthetist and have some extra tests before surgery.

    If you are struggling with Synopsis you can contact your pre-operative assessment clinic and they will be able to hopefully guide you through the process.

    If you have a clinical query relating to your operation (i.e. you no longer need the operation) you should contact your surgeon's secretary.

  • I have already completed a Synopsis questionnaire. Should I complete another one?

    I have already completed a Synopsis questionnaire. Should I complete another one?

    If you have already completed a Synopsis questionnaire for us (for example for another operation) then you will need to update it, especially if your medical details have changed.

    To do this, please login using the email address and password you registered with originally. You can update your details. Please be sure to share the updated questionnaire with us again.

    All the data you enter is treated confidentially and will not be shared. It is only viewable only by NHS staff at Worcestershire Acute Hospitals NHS Trust.
