Safer/Red2Green aims to transform how someone goes through their patient journey in our hospitals and beyond.

We want our staff to use the SAFER process to help speed up how quickly patients can be sent back home when they are well enough. The SAFER process is a method used across the country to reduce any delays for patients in adult inpatient wards; it aims to give all patients an expected date and the clinical criteria for their discharge from hospital.

We are also using Red2Green to encourage ward staff to discharge patients as early in the day as possible on the day they are fit to go home, which allows more time for any follow-up care or packages to be put in place.

This part of HomeFirst Worcestershire will be led by our Chief Nurse, Vicky Morris along with our dedicated Matron for Patient Flow, Kirsty Sloan.
