Frailty Assessment Unit (FAU)

Patients arriving at hospital, requiring assessment will be seen at our frailty assessment unit. 

The Frailty Assessment Unit based at the Alexandra Hospital, Redditch is part of a radical overhaul of the way we care for frail, older people. The new approach aims to cut down on the length of time people have to stay in hospital, the number of ward moves and readmissions.  This greatly improves the patient experience and means that older people are able to return home much quicker and maintain their independence. 

The service is open from 08:00 -17:00 Monday to Friday and from 08:00-14:00 Saturdays and Sundays

Who can use the service?

Who can use the service?

  • Group 1: Patients over 90 years of age

  • Group 2: Patients aged 75 and over, with 2 or more of the following geriatric giants:
    • IMMOBILITY - (movement disorders, Parkinson’s Disease, new or worsening)
    • INCONTINENCE - (new or worsening)
    • INSTABILITY (including falls and frailty)
    • INTELLECTUAL IMPAIRMENT (dementia and delirium, excluding learning disabilities)
    • IATROGENESIS (polypharmacy – on 5 or more medications)

  • Group 3: Patients aged 65 or over, from a Nursing Home, Residential Home or Community Hospital.

All patients meeting the frailty criteria (below) should be referred/conveyed to the Alexandra Hospital 24 hours a day 7 days a week.

Outside of the Frailty Assessment Unit opening hours frailty patients should be conveyed to the Emergency Department at the Alexandra Hospital. If the patient needs to stay in hospital they will be reviewed by the frailty team the following morning.

The Frailty Assessment Unit is open between the hours of 9am – 5pm Monday to Friday ( last patient accepted at 4pm) and 8am – 2pm Saturday and Sunday ( last patient accepted at 1pm.) During these hours patients should be conveyed directly to the Frailty Assessment Unit

During its opening hours GP’s and West Midlands Ambulance Service are encouraged to call the Frailty Assessment Unit to discuss the patient with the Unit’s staff

Meet the team

The Unit manager is Elaine Moreton and Matron Emma Welch. The unit is supported by a multidisciplinary team including doctors, nurses physiotherapists and occuptational therapists.
