NICE guidance states (CG87): 

‘Offer structured education to every person and/or their carer at and around the time of diagnosis, with annual reinforcement and review. Inform people and their carers that structured education is an integral part of diabetes care.’

Within Worcestershire a range of nationally recognised Diabetes Structured Education options are available.

  • First Steps
  • X-PERT Diabetes
  • X-PERT Insulin
  • DAFNE (Dose Adjustment for Normal Eating)
  • Carbohydrate Counting

Patients who have already attended a programme should be strongly encouraged to attend an annual update session.

Pathway for newly diagnosed type 2 diabetes

Step 1: patient is diagnosed with type 2 diabetes

  • Patient is given an information leaflet: ‘Living with Type-2 Diabetes’
    (further packs can be obtained from: wah-tr.worcsdiabetesed@nhs.net).
  • Patient is given access to healthy living with type 2 diabetes (online).
  • Patient is referred to First Steps.

Step 2: First Steps

  • Patient attends a one-off two-hour group education session, a range of dates including evenings and/or weekends is provided. Sessions are normally virtual and a limited range of face-to-face sessions are available.
  • Patient learns more about their diabetes and has opportunity to work through their information pack.
  • Patient finds out more about managing their diabetes and options for learning more.

Step 3: Patients choose how to manage their diabetes

Patients will be given three options for continuing their learning and can choose more than one option.

  1. X-PERT Diabetes – the group education programme, 2 ½ hours a week, lasting six weeks.
  2. Healthy living with type 2 diabetes – Activate /continue with the online support for diabetes self-management, with structured education and learning and self-management tools for goal setting, healthy eating, and more.
  3. Self-directed learning / individual support – books, internet, apps, support from their usual nurse or GP.
